Thought Someone Did a Good Job? Have an Idea to Help Us Improve? Got a Complaint?

Customer service is VERY IMPORTANT to me in our organization. As practice manager, I would want every encounter with our physician and non-physician providers to turn out wonderful and without any problems, I recognize that we don’t always reach this goal. This form is to provide you with an easy way to tell me when we did or did not meet your expectations – or you have an idea that you think can help us improve. Please take a moment to tell me about it.

There are a couple of things that I would want to caution you on….

  • I cannot respond to your ideas or issues without specific information. I must be able to reach you. Please provide a contact name, a phone number, and as many details as you can recall about the incident and what the actual issue was or the suggested improvement idea we could use.
  • Because we are not using a secure email protocol, please understand that any medical information provided might be seen by an unintended party. If this issue relates to your medical treatment, please simply state that you had a problem with the medical treatment without further details and we will contact you personally for more information to protect your confidentiality and health information.
  • All suggestions and comments will be used by Ashland ENT, Allergy & Hearing Aid Center to help us meet our goal to provide a great patient experience in future encounters and improves operations. Your submission acknowledges this intent and grants us permission to use the message to this end.
